First Visit

Your initial appointment will usually consist of an examination and review of your treatment options. Often a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable.  We have a request form for you to fill out before your first visit so that we may have these available.  
  • A list of medications you are presently taking.
  • If you have dental insurance, bring the necessary information (insurance card). This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e.,artificial heart valves, and recent joint replacements (within 3 years), rheumatic fever with heart damage, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office.   We also have an x-ray request form for you to fill out before your first visit so that we may request for you, and have the x-rays available at your first appointment.  Please print, fill out and sign the form.  You may then scan and email the completed form, fax, or deliver it to us in person.  We will take care of the rest.  

Prior to your first visit, we would ask that you visit our patient registration page to complete your medical history questionnaire, as well as an x-ray request form for any previous x-rays.
